COMMITTEE MEMBER APPLICATION

This application is used by the BSA to ensure uniform review and informed selection of committee member applicants. All applicants must be registered members of the Bloom Syndrome Association. One application per committee.  Before submitting this form, applicants should read and understand the Committee Member Agreement. Any additional correspondence regarding this application should be sent via email to the BSA.
